Join The Practice


All new patients registered with the practice will be asked to complete a number of forms, and they are available via the links below. Please complete these forms, email them ahead of time if you can, and if not bring into to Practice in person with Photo ID. All forms need to be completed prior to bringing them in or emailing them through in order for us to complete the registration within our clinical system. You may indicate on your registration form any preference for a particular doctor, with the practice.


Do you live within our boundary area?

Every GP practice has a specific registration area and our area map is displayed below. We will happily register any patient who lives in this area, please note you may be asked for proof of address. If you move, but remain in our practice area we may be able continue to be your GP, please let reception know of your new contact details. 



Please complete both forms (below) and either bring into to Practice in person with Photo ID. You may email them to prior to your presenting at the surgery with your ID. Otherwise you can complete the forms in the surgery.  Ask at the front desk for any assistance, and look to get Patient Access that will allow you to do a range of things without having to come into the practice.

Patient Questionnaire - The lets us know a number of important details about you, and there is a section where you can sign-up for email and tet messages with up to date information for you.

Summary Care Record is the form that allows us to share your medical information across the various services in Oxfordshire and nationally. This form is crucial, and registration cannot proceed without this.

In Oxfordshire, there is a specific version of the Summary Care Record, and an explanation is  available here.

Email your completed forms to this addess: when you are done, and then bring in some form of photo ID so we can identify you when you come in.

Practice Area

Our practice area covers the eastern part of the city and surrounding area and includes, Abingdon Road and surrounds, East Oxford, Iffley Fields, Cowley, Littlemore, Rose Hill, St Clements and parts of Headington and Marston. A wide range of social and ethnic groups are represented and translators can be booked for most languages.We also cater for students.
